About the Role

We are seeking an experienced Occupational Therapy Care Manager to join our Community Independence Service (CIS) team. As an integral part of our Reablement Team, you will play a crucial role in facilitating early supported discharge from hospital by delivering skilled Occupational Therapy interventions to individuals in their own homes.

Key Responsibilities
  • Deliver goal-oriented Occupational Therapy interventions to promote health and independence in individuals with disabilities
  • Assess eligibility for long-term care and support, and develop personalized plans to achieve optimal outcomes
  • Work closely with Reablement care providers to ensure seamless transitions and effective care coordination
  • Manage a caseload, prioritize tasks, and maintain accurate records of assessments, interventions, and contacts
  • Participate in duty OT rotations, responding to emergency visits and covering allocated cases
About You

We are looking for a highly skilled and motivated Occupational Therapist with a degree in Occupational Therapy, HCPC registration, and experience working in a multi-disciplinary setting with adults with disabilities. You will possess excellent communication, interpersonal, and problem-solving skills, with the ability to work independently and as part of a team. Proficiency in a range of IT programs, including Mosaic, System1, and MS Office, is essential.
