Job Summary

Spencer Clarke Group is seeking an experienced Occupational Therapist to join our Reablement Team in a community-based setting. As an Occupational Therapist, you will play a vital role in helping individuals with disabilities or those recovering from injury to build confidence and skills needed to stay safe and independent at home.

Key Responsibilities:

  • Conduct comprehensive functional assessments to identify individual needs and develop tailored reablement programs.
  • Collaborate with multidisciplinary teams to set and review goals, ensuring customers can safely manage daily activities with maximum independence.
  • Complete discharge assessments to identify ongoing needs and eligibility for continued support in line with the Care Act 2014.

Requirements:

  • Must have a Diploma/BSc in Occupational Therapy or equivalent and be registered with the Health and Care Professions Council (HCPC).
  • Knowledge and understanding of care coordination and the Trusted Assessor role within a social care/health setting.
  • Basic understanding of conditions leading to disability and the services available to support those affected.
