About the Role

As a Band 4 Urgent Care Co-ordinator, you will be responsible for delivering a comprehensive co-ordination service, providing business support, administrative leadership, and general office management.

Key Responsibilities
  • Deliver a high-quality co-ordination service, ensuring effective communication and collaboration with internal and external stakeholders.
  • Manage own workload, prioritising tasks, and meeting deadlines while maintaining accuracy and attention to detail.
  • Process Admission Avoidance referrals, gathering information, and signposting to relevant services.
  • Liaise with multidisciplinary teams, care providers, and external partners to facilitate smooth referral processes.
  • Monitor and maintain accurate records, using EPIC, CareFirst 6, and other relevant systems.

This role requires strong organisational and time-management skills, with the ability to work effectively in a fast-paced environment.
