About the Team

Our team is dedicated to supporting adults to live independently in their own homes. We are seeking an experienced Adult Reablement Journey Coordinator to join our team and help us achieve our goal of promoting independence.

Key responsibilities include:

  • Coordinating and delivering reablement services to support adults in their own homes.
  • Working closely with customers, families, and carers to understand their needs and develop personalized support plans.
  • Collaborating with healthcare professionals to ensure effective care coordination.

The ideal candidate will have experience in a similar role, with excellent communication and interpersonal skills. A full driving license and access to a vehicle is essential for this role.

Estimated Salary: £30,000 - £40,000 per annum
