This is an exciting opportunity to join our team as a Social Care Specialist in Derby City. As a Social Care Specialist, you will work closely with patients to address non-medical issues that affect their health and wellbeing.

The successful candidate will have a strong background in social care, experience working with people on a one-to-one basis, and excellent communication skills. A driving license and ability to travel around Derby efficiently are also essential for this role.

About the Role

We are looking for a skilled and compassionate individual to join our team as a Social Care Specialist. The ideal candidate will have a degree level qualification or similar in a health or care discipline, and relevant experience working in primary care or a community setting.

Your key responsibilities will include coproducing 'social prescriptions' with patients, addressing non-medical issues such as housing, debt, and social isolation. You will also work closely with other healthcare professionals to provide holistic support to patients.

Requirements

To be successful in this role, you will need to have:
* GCSEs or above
* Degree level qualification or similar in a health or care discipline
* Experience working with people on a one-to-one basis
* Excellent communication skills
* Ability to travel around Derby efficiently
* Driving license
