Job Description

This is an exciting opportunity for a skilled Registered Nurse to join our team as a Clinical Nurse Practitioner in sub-acute medicine. You will be responsible for assessing patient care needs, developing and implementing evidence-based programmes of care, and working collaboratively with multidisciplinary teams to deliver high-quality patient outcomes.

You will work closely with Ward Sisters and junior staff to support the assessment, planning, implementation, and evaluation of patient care, ensuring that care plans are individualised and adhered to. Your experience in sub-acute medical and rehabilitation patient care will be valuable assets in this role.

Responsibilities:
  1. Assess patient care needs and develop evidence-based programmes of care.
  2. Collaborate with multidisciplinary teams to deliver high-quality patient outcomes.
  3. Work closely with Ward Sisters and junior staff to ensure effective patient care.
  4. Maintain accurate records and contribute to quality improvement initiatives.
Requirements:
  • Degree in Nursing or equivalent qualification.
  • 1st level registration with the NMC.
  • Experience in sub-acute medical and rehabilitation patient care.
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
