NHS Greater Glasgow and Clyde is a leading healthcare provider, employing around 40,000 staff across various clinical and non-clinical professions.

We deliver a wide range of services including acute hospital, primary, community, and mental health care to a population of over 1.15 million.

The shift pattern for this role will be determined on an individual basis.

This full-time position involves working up to 37 hours per week.

Fixed-term employment for 12 months with the possibility of secondments.

An exciting opportunity has arisen to contribute to the delivery of Type 2 Diabetes care within NHS Greater Glasgow and Clyde as a Practice Nurse or Diabetes Specialist Nurse.

The successful candidate will be responsible for delivering the Type 2 Diabetes education programme 'Control it Plus' for newly diagnosed patients.

They will work closely with Diabetes Specialist Nurses and Primary Care support teams to promote self-management and Diabetes care.

In partnership with other healthcare professionals, they will develop and implement programmes of specialist diabetes education.

Hosted within the South Glasgow DSN team, the post holder will provide diabetes management, treatment, and education services to enable people with diabetes to effectively self-manage their condition.

Working autonomously in nurse-led clinics, patient homes, and community settings, they will assess, plan, and implement individualized patient care plans using a person-centered approach.
