Job Summary
Band 4 Assistant Practitioner Nursing
We are seeking a motivated and enthusiastic individual to join our Community Nursing team in Bedfordshire as a Band 4 Assistant Practitioner. This is a substantive post for the Mid Beds Community Nursing Team, delivering high-quality care to patients in their own homes.
Main Duties of the Job
You will work corporately within our Primary Care Team, supporting Community Nurses and the wider MDT to provide high-quality care to housebound residents with complex health conditions. Experience in adult community services is desirable, but a willingness to learn and contribute is more important.

Job Responsibilities
The successful candidate will be expected to be flexible and assist colleagues to cover service needs, including working across the wider integrated team. Access to a car is essential for service duties. A full induction program will be provided.
Person Specification

- GCSE Maths or equivalent
- Diploma level relevant qualification or equivalent experience

- Demonstrates appropriate emotional self-regulation and maintains professional boundaries at all times
- Good experience of working with community healthcare teams
- Experience of working with people on curative and/or palliative pathways
- Experience of working with the public demonstrating good customer care skills
- Experience of health coaching and promoting independent living
- Experience of working with people with physical health requirements
- Experience of managing a caseload of patients
- Experience of working with people with learning difficulties and/or mental health issues
- Experience of working without direct supervision
Knowledge and Skills
- Essential
- Demonstrates good understanding of principles of consent and ensures valid consent is obtained prior to undertaking nursing and care procedures
- Demonstrates good understanding of the Mental Capacity Act/Deprivation of Liberties and applies principles to everyday practice seeking advice/guidance as required
- Ability to prioritise work and work in a time-managed and effective way to meet patients' appointments and demands of the role
- Comprehensive clinical skills (venepuncture, Doppler, wound care, continence care, etc.)
- Ability to manage a caseload of patients
- Basic ability to use Microsoft packages - Word, Excel - and databases
- Able to support clients, carers, and family during times of emotional distress
- Be able to undertake frequent standing, kneeling, crouching, hoisting, and other moving and handling procedures
- Regularly move and handle clients that may require moderate physical effort, in confined spaces and often unsuitable environments, using relevant practical experience, equipment, and skills and enable optimum safety and best practice
