About the Role:


We are seeking an experienced Community Nurse to join our team as a Nurse Practitioner in one of our Primary Care Networks (PCNs) in Surrey Downs Health & Care.


The successful candidate will have a strong background in community health nursing and a passion for delivering high-quality patient care.


Key Responsibilities:



  • Assess and develop care plans for patients in their homes and community settings
  • Work closely with multidisciplinary teams to coordinate care and support
  • Provide education and training to patients, families, and healthcare professionals
  • Contribute to the development and implementation of community health services and initiatives

Requirements:



  • Registered Adult Nurse with NMC
  • Valid Driving Licence (to drive within UK)
  • Teaching qualification
  • Evidence of continued professional development post registration
  • Access to a vehicle for work purposes
  • Role model to others in both clinical and team cohesiveness
  • Evidence of experience of team management and multi-disciplinary working
  • Relevant post registration experience
