An exciting opportunity has arisen for a registered nurse, other healthcare professional or social worker to work across the Bracknell Community Mental Health Team and substance misuse service, New Hope. This is a Band 6 role.

We are looking for a registered professional who is driven, passionate and excited to be working with and linking two essential community services to make a real difference to our service users' lives.

The successful candidate will support a caseload of individuals with complex mental illness and substance misuse difficulties. They will also help to support colleagues in both teams to understand the complex interplay of mental health and substance misuse by carrying out assessments and contributing to care plans and Recovery Plans.

Main Duties
  • To support a caseload of individuals with mental illness and substance misuse difficulties.
  • To use appropriate, evidence-based, therapeutic interventions to address the co-occurring mental health, alcohol and substance use needs of service users and their families.
  • Assessment of risk and care needs in line with both team's models of work and recovery. To use these assessments to then inform the care and support that is provided to the individual.

Requirements
  1. A registered nurse, HCP or social worker with a recognised degree-level qualification and professional registration.
  2. Evidence of continuing professional development.
