Job Title: Community Health Specialist

Job Description

We are seeking a dedicated Community Health Specialist to join our team at Leaders In Care Recruitment Ltd. As a Community Health Specialist, you will be responsible for providing direct nursing care in the community, working closely with families to develop personalised care plans, and partnering with other healthcare professionals to ensure optimal patient care.

Key Responsibilities:

  • Providing direct nursing care in the community
  • Working closely with families to develop personalised care plans
  • Partnering with other healthcare professionals to ensure optimal patient care
  • Ensuring accurate record-keeping and reporting
  • Managing your own caseload, demonstrating excellent time management and organisational skills

The ideal candidate should have:

  • Competency with IV therapies
  • A Registered Nurse qualification
  • Previous experience in a community nursing role
  • A valid driver's license
