About the RoleThis is a 16-hour per week position, working 2 days per week (Monday and Wednesday). You will be responsible for managing programmes of care for patients with long-term conditions, providing specialist nursing care, and working collaboratively with our multidisciplinary team.
Key Responsibilities- Deliver high-quality care to patients with long-term conditions, including asthma, COPD management, diabetes, and chronic heart disease.
- Manage programmes of care for patients with long-term conditions, planning, providing, and evaluating care under agreed local and national guidelines.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Participate in clinical meetings and contribute to the development of clinical guidelines and protocols.
- Ensure accurate and concise documentation of patient care and outcomes.
To be successful in this role, you will need:
- Current NMC registration and 1st Level registration.
- Relevant long-term condition management qualification or experience.
- Evidence of recent Continual Professional Development.
- Cytology and relevant Vaccination and Immunisation training.
