Job Summary

We are seeking a skilled Long Term Condition Nurse to join our team at Cumbria Health. As a key member of our primary care team, you will play a vital role in delivering high-quality care to our patients with long-term conditions.

About the Role

This is a 16-hour per week position, working 2 days per week (Monday and Wednesday). You will be responsible for managing programmes of care for patients with long-term conditions, providing specialist nursing care, and working collaboratively with our multidisciplinary team.

Key Responsibilities
  1. Deliver high-quality care to patients with long-term conditions, including asthma, COPD management, diabetes, and chronic heart disease.
  2. Manage programmes of care for patients with long-term conditions, planning, providing, and evaluating care under agreed local and national guidelines.
  3. Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  4. Participate in clinical meetings and contribute to the development of clinical guidelines and protocols.
  5. Ensure accurate and concise documentation of patient care and outcomes.
Requirements

To be successful in this role, you will need:

  • Current NMC registration and 1st Level registration.
  • Relevant long-term condition management qualification or experience.
  • Evidence of recent Continual Professional Development.
  • Cytology and relevant Vaccination and Immunisation training.
