Job Summary: We are seeking a highly motivated and organized individual to join our team as a Care Coordinator. As a Care Coordinator, you will be responsible for providing stimulating and meaningful activities for our residents, while promoting their privacy, dignity, and independence.

Key Responsibilities:

  • Develop and implement activity programs that cater to the diverse needs and interests of our residents
  • Collaborate with the care team to ensure that activities are tailored to meet the individual needs of each resident
  • Lead and organize activities, including chair exercises, reminiscing sessions, and arts and crafts
  • Accompany residents on outings, such as the local country fair
  • Provide support and guidance to residents to help them participate in activities

Requirements:
