About the Role

We are seeking a skilled and compassionate Associate Practitioner to join our Crisis Resolution Home Treatment Team. As a key member of our team, you will play a vital role in providing support to individuals in crisis, promoting their recovery and independent living in the community.

Key Responsibilities
  • Conduct assessments and develop care plans to address the needs of service users.
  • Work collaboratively with multidisciplinary teams to provide holistic support and interventions.
  • Develop and maintain relationships with service users, their families, and carers to ensure a comprehensive understanding of their needs.
  • Provide therapeutic interventions and support to promote recovery and independent living.
  • Contribute to the development and implementation of care plans, ensuring they are tailored to the individual's needs and goals.

Requirements
  • Foundation degree or NVQ in Health and Social Care level 3 or equivalent experience.
  • Proven knowledge and skills in caring for individuals with mental ill health.
  • Experience of working with mental ill health and understanding of related issues.
