About the RoleWe are seeking a skilled Nurse Specialist to join our team in Wiltshire. As a Band 6 Health Visitor, you will be responsible for delivering high-quality, person-centered care to individuals and families in the community.
Your primary focus will be on promoting health, preventing illness, and empowering clients to take control of their own well-being.
You will work closely with other healthcare professionals to provide comprehensive care that meets the unique needs of each client.
In this role, you will have the opportunity to make a real difference in people's lives and contribute to the delivery of excellent patient outcomes.
Key Responsibilities- Deliver high-quality, person-centered care to individuals and families in the community
- Work collaboratively with other healthcare professionals to provide comprehensive care
- Empower clients to take control of their own well-being and promote healthy lifestyles
- Conduct home visits and assessments to identify client needs and develop personalized care plans
To be successful in this role, you will need:
- A recognized qualification in health visiting (SCPHN)
- Valid UK driving licence and access to a vehicle
- A passion for delivering high-quality, person-centered care
- A drive to continuously develop professionally and expand your skill set
