About the RoleThe ideal candidate will have a strong background in occupational therapy, with experience in rehabilitation and condition management. You will be responsible for assessing patients, planning and implementing effective interventions, and working closely with healthcare professionals and social care teams to ensure seamless care.
Key Responsibilities- Conduct comprehensive assessments of patients in community settings to identify their needs and develop personalized care plans.
- Plan and implement evidence-based interventions to promote independence, mobility, and functional ability.
- Collaborate with multidisciplinary teams to ensure coordinated care and effective communication.
- Supervise and mentor junior staff, students, and therapy support staff to ensure high-quality care.
- Participate in service development, quality improvement initiatives, and research projects to enhance patient outcomes and service delivery.
- Degree in Occupational Therapy and HCPC registration.
- Post-registration experience in rehabilitation and condition management, with a strong background in occupational therapy.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to work independently and as part of a team, with a strong commitment to patient-centered care.
- Good analytical and problem-solving skills, with the ability to prioritize tasks and manage a caseload effectively.
