About the Role:

We are seeking an experienced Care Coordinator to join our dedicated team at Apollo Home Healthcare. As a Care Coordinator, you will play a key role in the continuous delivery of care to our service users.

Key Responsibilities:

  • Manage a caseload of care packages and coordinate care delivery with our care teams.
  • Apply your communication and relationship building skills to effectively manage rotas and respond to day-to-day changes.
  • Act as the first point of contact for service users, their families, and other healthcare professionals.
  • Work collaboratively with our care teams to ensure seamless care delivery.
  • Support the out-of-hours on-call service on a 1-in-5 rota basis.

Requirements:

  • Experience in care coordination and management.
  • Strong communication and interpersonal skills.
  • Ability to work well under pressure and adapt to changing situations.
  • Valid driving license and access to a vehicle.

What We Offer:

  • Competitive salary between £29,700 - £34,700 per annum.
