About the Role

We are seeking a skilled and experienced Advanced Support Worker to join our team in Worcester. As a key member of our team, you will play a pivotal role in supporting individuals with learning disabilities, autism, and complex needs to live their best lives possible.

Key Responsibilities
  • Support individuals with learning disabilities, autism, and complex needs to lead independent lives
  • Develop and implement personalized support plans to meet the unique needs of each individual
  • Work collaboratively with colleagues and external agencies to ensure seamless care and support
  • Contribute to multi-agency care and support planning to ensure the best possible outcomes for individuals
  • Participate in regular reflective practice sessions to ensure the best strategies are always utilised for the people supported
Requirements
  • Level 3 in Social Care or equivalent experience
  • Strong experience in mental health, autism, and forensic backgrounds
  • Ability to connect with people and be passionate about supporting individuals to live independently
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively as part of a team
  • Flexibility and adaptability to meet the changing needs of individuals
