Key Responsibilities- Support individuals with learning disabilities, autism, and complex needs to lead independent lives
- Develop and implement personalized support plans to meet the unique needs of each individual
- Work collaboratively with colleagues and external agencies to ensure seamless care and support
- Contribute to multi-agency care and support planning to ensure the best possible outcomes for individuals
- Participate in regular reflective practice sessions to ensure the best strategies are always utilised for the people supported
- Level 3 in Social Care or equivalent experience
- Strong experience in mental health, autism, and forensic backgrounds
- Ability to connect with people and be passionate about supporting individuals to live independently
- Excellent communication and interpersonal skills
- Ability to work collaboratively as part of a team
- Flexibility and adaptability to meet the changing needs of individuals
