Job Title: Registered Mental Health Nurse

Job Summary:

We are seeking a highly skilled Registered Mental Health Nurse to join our Memory Assessment Service team in Morecambe. As a key member of our multidisciplinary team, you will be responsible for assessing, planning, and delivering high-quality care to patients with dementia.

Key Responsibilities:

  • Conduct comprehensive assessments of patients with dementia, identifying their needs and developing personalized care plans.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and effective risk management.
  • Provide expert guidance and support to patients, their families, and carers, promoting independence and well-being.
  • Participate in multidisciplinary team meetings to discuss patient progress and develop care strategies.
  • Contribute to the development and implementation of service policies and procedures, ensuring compliance with regulatory requirements.

Requirements:

  • Registered Mental Health Nurse (RMN) with experience in dementia care.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work autonomously and as part of a team, with a flexible approach to work patterns.
