We are currently seeking a highly skilled and experienced Qualified Social Worker to join our client's Ongoing Support Team. As a key member of the team, you will be responsible for delivering high-quality social care services to individuals and families.

The successful candidate will have a proven track record of working within an Ongoing Support setting within the UK statutory sector, with a minimum of 1 year post-qualifying experience. You will be registered with the HCPC and hold Enhanced DBS/CRB clearance or be eligible for it. Additionally, you must have the right to work in the UK.

In this role, you will be responsible for creating and carrying out reviews of care packages, undertaking comprehensive social care assessments and reports, and working with families, carers, and communities to help them make informed decisions. You will also assess and reassess complex health and social care needs, request funding, and complete Decision Support Tools to establish eligibility for healthcare.
