Job Title: Community Care Professional

Job Summary: We are seeking a highly motivated and experienced Community Care Professional to join our team at Rainbow Care Services. In this role, you will be responsible for providing high-quality care and support to our service users, enabling them to live independently in their community.

About the Role:
As a Community Care Professional, you will perform a range of tasks, including assessing the needs of our service users, developing and implementing care plans, and providing emotional support and companionship. You will also collaborate with healthcare professionals and other members of the multidisciplinary team to ensure that our service users receive comprehensive and coordinated care.

Key Responsibilities:
* Assess the needs of service users and develop individualized care plans
* Provide personal care in a sensitive and culturally appropriate manner for all service users
* Monitor and evaluate the effectiveness of care plans and make recommendations for improvement
