Role Overview

Our team at JLKare and Support Limited is seeking a compassionate and dedicated Care Coordinator to join our community-based support services. This role involves guiding and supporting a team while providing high-quality care to individuals. Key responsibilities include:

Key Responsibilities:
  • Conduct regular staff supervisions and lead productive team meetings.
  • Act as a key point of contact between our team, families, advocates, and other professionals.
  • Regularly update care plans and risk assessments to ensure the highest standards of care.
  • Oversee the management of medication, including completing Medication Competency Assessments.
  • Compile monthly reports on client progress and service outcomes.
  • Be available for on-call duties 3 days per month at £50 per day.
