About the Role

We are seeking a highly skilled and compassionate Senior Healthcare Support Worker to join our Community Nurse teams in a rural/coastal area. As a key member of our team, you will provide nursing care to patients in their own homes or residential care environments, working under the direction of the Nurse (Community) and Nurse Specialist (Community).

Key Responsibilities
  • Provide high-quality nursing care to patients in their own homes or residential care environments.
  • Work collaboratively with the Community Nurse team to assess care needs and deliver planned care under delegation and direction of the Registered Nurse.
  • Contribute to the development of care plans and provide support to patients and their families.
  • Participate in training and development opportunities to enhance your skills and knowledge.
  • Work effectively as part of a team to achieve high-quality patient outcomes.
Requirements
  • Level 3 Diploma/NVQ 3 in Health and Social Care or equivalent qualification.
  • Experience of working with older people in a community setting (desirable).
  • Competent with IT, including electronic recording systems.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
