We are seeking an experienced Advanced Clinical Research Practitioner to join our team at West Kent Primary Care. As an Advanced Clinical Research Practitioner, you will play a key role in delivering high-quality patient care and contributing to the development of our primary care services.
About the RoleThe successful candidate will be responsible for working in our Same Day Access Hub, seeing patients for urgent on-the-day issues. You will work alongside our PCN team of GP supervisors and paramedic practitioners to deliver safe and high-quality care. Your duties will include assessing, diagnosing, planning, implementing, and evaluating treatment and interventions for patients presenting with acute, long-term, and complex health needs.
Key Responsibilities- Assess and diagnose patients with acute, long-term, and complex health needs
- Develop and implement treatment plans in collaboration with patients, carers, and other healthcare professionals
- Work effectively with individuals in other agencies to meet patients' needs
- Manage and lead the delivery of specifically identified services or projects as agreed with the PCN Clinical Director and the PCN management team
- Undertake mentorship for more junior staff, assessing competence against set standards
- Use evidence-based strategies to improve and enhance the quality of patient care
- Master's degree level in the relevant area of expertise
- Advanced clinical practice skills, including clinical examination skills and accountability of own role and other roles
- Experience in general practice, elderly care, or palliative care
- Ability to identify determinants of health in the area and knowledge of public health issues
- Highly motivated and flexible with excellent communication and leadership skills
