We are seeking an experienced and skilled Community Nurse Practitioner to join our team at Torbay and South Devon NHS Foundation Trust. As a key member of our Integrated Care Team, you will play a vital role in delivering high-quality patient care and contributing to the development of our services.
Key Responsibilities- Provide comprehensive assessments and care planning for patients in their own homes, taking into account their physical, social, and emotional needs.
- Work autonomously to plan and implement nursing treatment programs, collaborating with other healthcare professionals as needed.
- Advise and educate patients and carers on health and wellbeing issues, promoting self-care and independence.
- Conduct specialist and shared complex nursing assessments, including medication reviews, prescribing, and palliative care.
- Utilize advanced nursing skills and knowledge to ensure high-quality patient care, adhering to Trust policies and NMC standards.
- Monitor and evaluate care programs, promoting effective coordination and continuity of patient care.
- Formulate and implement nursing care plans in collaboration with patients and their carers, ensuring evidence-based practice and sound clinical judgment.

Requirements- Qualified Registered Nurse Level 1
- BSc (Hons) in Community Health Nursing (District) or equivalent
- Independent/Supplementary Prescriber or commitment to undertaking this course within 3 years
- Mentorship qualification or experience, or commitment to undertaking an appropriate mentorship course
- Previous community experience or experience of lone working
- Experience of working within multidisciplinary teams to influence high-quality care programs for patients with complex health needs
- Evidence of working at a senior level in community nursing, admission avoidance, unplanned care, or primary care experience
- Holistic client assessment skills using an identified model of care
- Ability to make sound clinical judgments and decisions in response to complex or crisis situations
- Autonomy and self-motivation, with a willingness to work flexibly and adapt to changing priorities
- Excellent communication and interpersonal skills, both written and oral
- Liaising with and developing effective partnerships with a wide range of individuals and organizations
