We are seeking an experienced and passionate Advanced Practitioner Social Worker to join our dedicated Adult Community Learning Disabilities Team in the South East.

About the Role

As a key member of our team, you will be responsible for carrying out Care Act assessments, safeguarding work, multi-agency work, and duty functions within the team. You will work in accordance with social care legislation and local government requirements, informed by the values and principles of social care.

Key Responsibilities
  • Carry out Care Act assessments and make decisions about the most appropriate interventions.
  • Conduct safeguarding work and multi-agency work to ensure the well-being of adults with learning disabilities.
  • Work in accordance with the Care Act 2014, Mental Capacity Act 2005, Human Rights Act, and Mental Health Act.
  • Support the delivery of person-centred, safe, and effective social care services.
Requirements

To be successful in this role, you will need:

  • Previous experience in working in Adult Social Care.
  • A good understanding of social care legislation and local government requirements.
  • Knowledge of the Care Act 2014, Mental Capacity Act 2005, Human Rights Act, and Mental Health Act.
  • Ability to work in a specific area within the Authority and carry out duty functions within the team.
