The Role

We are seeking an experienced Rehabilitation Assistant to join our team in Central London. As a key member of our community-based service, you will play a vital role in supporting individuals with neurological conditions to achieve their full potential.

Key Responsibilities

  • Deliver person-centered rehabilitation programs to patients with complex needs.
  • Collaborate with multidisciplinary teams to develop and implement effective care plans.
  • Conduct assessments and evaluations to identify patient goals and objectives.
  • Provide education and support to patients and their families to promote independence and self-management.

Requirements

  • Degree or equivalent qualification in Health Studies or related field.
  • Experience working with patients with neurological conditions.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
