About the Role

We are seeking a highly skilled and experienced Senior Nurse Specialist in End of Life Care to lead our Emergency Department End of Life Service. As a key member of our team, you will be responsible for delivering high-quality, safe, and effective end of life care to patients and their families.

Key Responsibilities
  • Practice autonomously at a specialist level in the delivery of high-quality, safe, and effective end of life care, including a significant proportion of direct, higher-level clinical care with strategic leadership and management.
  • Work with patients, families, and carers, providing direct nursing care and clinical advice to the multidisciplinary team.
  • Lead a team of professionals, working closely with Trust initiatives and End of Life projects/transformation plans, aligning service delivery with Trust KPIs, in tandem with patient need.
  • Ensure the team facilitates better patient/carer experience of dying within the ED, where appropriate, to facilitate timely transfers of care to appropriate locations (community and bedded locations), in line with patient choice (preferred place of care and preferred place of death).
  • Actively coordinate and contribute to team efforts in facilitating specialist knowledge transfer by providing ad-hoc (working alongside colleagues) and planned education events/opportunities, in and outside of the ED (partner teams, internally and externally).
  • Be part of the clinical leadership team within the UHP Supportive and Palliative Care Service (across all elements of the service, as required).
  • Represent the Trust and present, provide reports, reporting on project deliverables, and be accountable for service KPIs.
  • Work closely, aligning service offer, with partner teams, internally and externally, including acute oncology, departments relating to urgent and emergency care, Virtual Ward, AGP (as examples) and with external stakeholders, such as primary care, St Lukes Hospice, care coordination hubs, Livewell and SWASFT (as examples).
  • Coordinate and co-deliver peripatetic function in relation to admission/conveyance avoidance; this will include on-site support of offsite UHP End of Life bed bases.
  • Design, coordinate, and support the delivery of a 7-day service professionals pre-conveyance telephone/virtual support advice service.

Key to Role

  • In-depth demonstrable knowledge and experience working with processes, services (community and acute setting) and pathways which support admission avoidance and enable preferred place of care/death.
  • The ability to practice autonomously using specialist skills of assessment, investigation, and the management in EoL patient care.
  • Non-medical prescribing qualification, or actively working towards.

Desirable

  • Leadership experience within the acute setting/managing teams.
  • Experience of community End of Life care and support.
  • Masters degree in a related subject.

Essential

  • Advanced Communication Skills Training.

Desirable

  • Advanced communication 'train the trainer' qualification.
