About the Role

This is an exciting opportunity to make a positive impact on the lives of children and young people in Middlesbrough. As an Outreach Practitioner, you will play a key role in supporting inclusion and building inclusive capacity in schools and settings.

Key Responsibilities
  • Contribute to the assessment, planning, and delivery of high-quality interventions for children and young people with Special Educational Needs (SEN) or those at risk of permanent exclusion.
  • Work closely with children, young people, families, staff, and other agencies to provide effective support.
  • Support the delivery of a 0-25 Outreach and Inclusion model, promoting inclusive learning and well-being.
Requirements
  • Knowledge and experience of supporting children with SEN.
  • Good understanding of how children and young people learn and strategies to support inclusive learning and well-being.
  • Ability to work effectively in multi-disciplinary teams and advise/coach others to overcome barriers to successful inclusion.
  • Organizational skills, prioritization, flexibility, and a solution-focused approach.
  • Good IT skills and excellent communication skills.
