Job Title: Outreach Practitioner

This is an exciting opportunity to join our team and make a positive impact on the lives of children and young people in Middlesbrough.

The Outreach Practitioner will work closely with the Inclusion Team to support the identification and assessment of children and young people who require additional support. The role involves contributing to the assessment, planning, and delivery of high-quality interventions for children and young people with Special Educational Needs (SEN) or those at risk of permanent exclusion.

The successful candidate will have:

  • Knowledge and experience of supporting children with SEN
  • Good understanding of how children and young people learn and strategies to support inclusive learning and well-being
  • Ability to work effectively in multi-disciplinary teams and advise and coach others to overcome barriers to successful inclusion
  • Flexibility, positive energy, a solution-focused approach, and resilience
  • Good IT skills and excellent communication skills

The role is part-time, working 32.5 hours per week, term-time only. The post is subject to an enhanced Disclosure and Barring Service (DBS) check.
