About the Role:

We are seeking a dedicated and compassionate individual to join our Community Reablement team as a Social Care Worker. As a key member of our team, you will play a vital role in supporting individuals to regain their independence and confidence in their daily lives.

Key Responsibilities:

  • Work in partnership with individuals in their own homes to help them regain their independence in carrying out daily tasks.
  • Support individuals to regain skills and confidence in areas such as cooking, medication management, and personal care.
  • Develop and implement personalized plans to meet the unique needs and goals of each individual.
  • Work collaboratively with other healthcare professionals to ensure seamless care and support.
