About the Role

We are seeking a dedicated and compassionate individual to join our Wellbeing Team as a Community Care Specialist.

As a key member of our team, you will provide personalized care and support to enable individuals to live well at home and be part of their community.

Our team challenges loneliness, boredom, and helplessness, supporting people to feel connected, active, and purposeful in their own way.

Key Responsibilities
  • Provide high-quality care and support to individuals, promoting their independence and well-being.
  • Work collaboratively with colleagues and external partners to ensure seamless delivery of services.
  • Maintain accurate records and reports, adhering to regulatory requirements.
Requirements
  • Proven experience in care work or a related field.
  • Demonstrated ability to work effectively with diverse populations and adapt to changing circumstances.
  • Strong communication and interpersonal skills, with a focus on empathy and understanding.
  • Basic knowledge of dementia, challenging behaviors, and manual handling techniques.
