General Practitioner

We are seeking a highly skilled and compassionate General Practitioner to join our team at Fownhope Medical Centre. As a Care Home GP, you will be responsible for providing high-quality medical care to our patients in a primary care setting.
Key Responsibilities:- Manage a caseload of patients and provide a wide range of health services, including surgery consultations, telephone consultations, and home visits.
- Make professional decisions in relation to presenting problems, whether self-referred or referred from other healthcare workers.
- Assess the health care needs of patients with undifferentiated and undiagnosed problems.
- Screen patients for disease risk factors and early signs of illness.
- Develop care plans for patients in consultation with them and in line with current practice disease management protocols.
- Provide counselling and health education to patients.
- Admit or discharge patients to and from the caseload and refer to other care providers as appropriate.
- Record clear and contemporaneous consultation notes to agreed standards.
- Collect data for audit purposes.
- Prescribe medications in accordance with the practice prescribing formulary.
- Awareness of and compliance with all relevant practice policies and guidelines.
- A commitment to lifelong learning and audit to ensure evidence-based best practice.
- Contributing to evaluation and audit and clinical standard setting within the organisation.
- Undertaking any other duty as reasonably requested by the partners to facilitate adequate provision of services to patients.
In the course of seeking treatment, patients entrust us with sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately.
Communication:- Communicate effectively with other team members.
- Communicate effectively with patients and carers.
- Recognize people's needs for alternative methods of communication and respond accordingly.
- Assist in promoting and maintaining their own and others health, safety and security as defined in the Practice Health & Safety Policy.
- Awareness of national standards of infection control and cleanliness and regulatory / contractual / professional requirements.
- Correct personal use of Personal Protective Equipment (PPE) and ensuring correct use of PPE by others.
- Management of the full range of infection control procedures in both routine and extraordinary circumstances.
- Act in a way that recognizes the importance of people's rights and interprets them in a way that is consistent with Practice procedures and policies.
- Respect the privacy, dignity, needs and beliefs of patients, carers and colleagues.
- Participate in an annual individual performance review.
- Take responsibility for own development, learning and performance.
- Alert other team members to issues of quality and risk.
- Assess own performance and take accountability for own actions.
- Contribute to the effectiveness of the team by reflecting on own and team activities.
