About the Opportunity

As a key member of our team, you will work collaboratively with GP and Welsh Ambulance colleagues to intervene at the earliest point of crisis, aiming to support individuals in avoiding conveyance to Accident & Emergency.

Main Duties

  • Collaborating with integrated Health and Local Authority community teams to deliver place-based care ethos.
