About the Role

We are seeking a highly skilled and motivated Respiratory Practitioner to join our Community Respiratory Service at Sussex Community NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to patients with chronic respiratory conditions.

Key Responsibilities
  • Assess and treat patients with complex respiratory needs, working collaboratively with the multidisciplinary team to achieve optimal outcomes.
  • Develop and implement individualized care plans, taking into account the patient's physical and psycho-social needs.
  • Work closely with other healthcare professionals to ensure seamless care transitions and effective communication.
  • Contribute to the development and implementation of service policies, protocols, and care pathways.
  • Participate in quality improvement projects and engage in opportunities for developing leadership qualities.
Person Specification

We are looking for a candidate with:

  • Significant experience in respiratory care, preferably in a community setting.
  • Strong clinical knowledge and skills in respiratory assessment and management.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and healthcare professionals.
  • A flexible and adaptable approach, with the ability to prioritize tasks and manage a caseload effectively.
  • A commitment to ongoing professional development and continuing education.
