To be successful in this role, you will need to possess excellent communication and interpersonal skills, with the ability to build strong relationships with clients, colleagues, and stakeholders. You will also require a passion for working with individuals in recovery, with a commitment to supporting them in achieving their goals and aspirations.
Key Responsibilities:
- Conduct detailed assessments to identify client needs and risks
- Develop and maintain personalized care plans outlining actionable steps for improvement
- Facilitate group interventions to promote psychosocial outcomes, resilience, and wellbeing
- Deliver evidence-based psychosocial programs to support long-term recovery and positive outcomes
- Provide integrated support addressing clients' mental, physical, and emotional needs
- Collaborate with healthcare and social care providers to ensure comprehensive care plans
Requirements:
- Experience in care planning, risk assessments, and multi-agency collaboration
- Strong communication and interpersonal skills
- Ability to work effectively within a team and independently
- Passion for supporting individuals in recovery
- Enhanced DBS dated within the last 12 months (or willingness to complete one)
Estimated Salary: £28,000 - £32,000 per annum, depending on experience
