Prison Leaver Support Specialist

The role you will be undertaking is a critical one, as part of our Critical Time Intervention (CTI) team working across Liverpool City Region. As a Prison Leaver Support Specialist, you will be responsible for providing person-centered support to individuals transitioning out of prison and into accommodation, thereby preventing homelessness on release from custody.
Crisis' CTI model is an evidence-based approach designed to navigate fragmented systems, utilizing a time-limited case management approach to support individuals during a period of transition. This four-phase model consists of Pre-CTI, Transition, Try-out, and Transfer of Care, each phase carefully defined to ensure seamless support throughout the process.
You will work within this framework, collaborating with stakeholders to empower homeless and vulnerably housed individuals. Your expertise in delivering coaching and mentoring, learning support, and personal development training will be invaluable in this role. Additionally, your understanding of issues affecting homeless or vulnerably housed individuals, such as mental illness, addiction, the benefit system, and housing, will enable you to provide tailored support.
We seek candidates with a strong appreciation for delivering support within a time-limited framework. A valid driving license and access to a vehicle are essential requirements due to the nature of the role.

In return for your dedication, you can expect a competitive salary of £28,000 - £30,000 per annum, depending on experience.
