The Wrightington Wigan and Leigh Teaching Hospitals NHS Foundation Trust is a leading healthcare provider seeking an experienced Community Nurse to join their Frailty SDEC team. This full-time role offers a 12-month fixed-term contract with the possibility of permanency, covering maternity leave.

About the Role

This position involves providing high-quality care to patients requiring short-term medical interventions and rehabilitation. The successful candidate will be responsible for supporting individuals to achieve specific goals, enabling safe discharge back into the community or their own home.

As a key member of the multi-disciplinary team, you will be expected to communicate effectively and participate in assessing, planning, implementing, and evaluating programmes of care. Your expertise will be essential in upholding NMC standards and adhering to Trust policies and procedures.

You will be required to work flexible shifts, including days and nights, as part of a 24/7/365 service. A positive approach to work, flexibility, and excellent communication skills are essential for this role.

Key Responsibilities
  • Provide high-quality patient care, adhering to NMC Code of Conduct.
  • Participate in assessing, planning, implementing, and evaluating programmes of care.
  • Communicate effectively with the multi-disciplinary team regarding patient care.
  • Assist with investigating and resolving complaints and PALS enquiries.

An estimated salary for this role is £32,000 - £43,041 per annum, depending on experience.
