Long Term Conditions Nurse

We are seeking a highly skilled and dedicated Long Term Conditions Nurse to join our clinical team at HCRG Care Group. As a key member of our team, you will play a vital role in delivering high-quality patient care and contributing to the success of our organization.
Key Responsibilities- Provide skilled nursing care to patients with long-term conditions, assessing their immediate and long-term needs and developing effective care plans.
- Lead responsibility for delivering primary care clinics in a clinically effective and timely manner, responding to individual patient needs.
- Participate in and promote nursing research and project work to enhance the delivery of skilled, high-quality care within the team.
- Treat all patients with dignity and respect, maintaining confidentiality and adhering to data protection policies.
- Collaborate with colleagues to ensure continuity of care between clinical settings, promoting effective communication and coordination.
- Maintain accurate, comprehensive, and contemporaneous records in accordance with HCRG Care Group procedures.
- Stay up-to-date with the latest clinical practices and guidelines, acting as a resource for other staff members.
- Develop and deliver teaching programs for staff at all levels, in conjunction with the senior team member and The Learning Enterprise.
- Identify staff training needs and facilitate a positive learning environment, promoting professional development and growth.
- Maintain professional and clinical integrity, adhering to NMC guidelines and standards.

- Registered Nurse with a degree-level qualification and NMC registration.
- Clinical experience in primary healthcare, A&E, or other community settings.

- Management of common minor injuries and illnesses, long-term conditions, and vaccination clinics.
- Excellent communication and interpersonal skills, with the ability to communicate complex information effectively.
- Knowledge of IT systems, including word processing, emails, and the internet.
- Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
