Key Responsibilities- Provide skilled nursing care to patients with long-term conditions, assessing their immediate and long-term needs and reviewing care plans within assigned clinics.
- Ensure the delivery of primary care clinics in a clinically effective and timely manner, responding to individual patient needs.
- Participate in and promote nursing research and project work within the clinical area to promote the delivery of skilled high-quality care within the team.
- Treat all patients with dignity and respect, involving and supporting them in their care.
- Communicate effectively with colleagues and other healthcare professionals to ensure continuity of care.
- Maintain accurate and comprehensive records of patient care, adhering to HCRG Care Group procedures.
- Stay up-to-date with the latest clinical practices and guidelines, acting as a resource for other staff members.
- Contribute to the development of teaching programs for all levels of staff, in liaison with the senior team member and training and learning enterprise.
- Identify staff training and development needs, facilitating a good clinical learning environment for all members of the nursing team.

- Registered Nurse with a degree-level or equivalent qualification and NMC registration.
- Clinical experience working in primary healthcare, A&E, or other community settings.
- Evidence of continual learning and professional development.
- Management of common minor injuries and illnesses, long-term conditions, vaccination clinics, and clinical examination skills.
- Knowledge of the needs of patients with long-term conditions to complete appropriate care plans.
- Excellent communication and interpersonal skills, with the ability to communicate complex and sensitive information effectively with people at all levels.
- Knowledge and understanding of health frameworks for primary care.
