About the Role

We are seeking a highly skilled and experienced Long Term Conditions Nurse to join our team at HCRG Care Group. As a key member of our clinical team, you will play a vital role in delivering high-quality care to our patients.

Key Responsibilities
  • Provide skilled nursing care to patients with long-term conditions, assessing their immediate and long-term needs and reviewing care plans.
  • Ensure that primary care clinics are delivered in a clinically effective and timely manner, responding to individual patient needs.
  • Participate in and promote nursing research and project work within the clinical area to promote the delivery of skilled high-quality care.
  • Treat all patients with dignity and respect, involving and supporting them in their care.
  • Communicate effectively with colleagues and other healthcare professionals to ensure continuity of care.
  • Maintain accurate and comprehensive records of patient care, adhering to HCRG Care Group procedures.
  • Stay up-to-date with the latest clinical knowledge and best practices, acting as a resource for other staff members.
  • Contribute to the development and implementation of teaching programs for all levels of staff, in collaboration with the senior team member and The Learning Enterprise.
