We are seeking a skilled and compassionate Support Worker to join our Children's Continuing Care Team at Great Western Hospitals NHS Foundation Trust. As a Support Worker, you will play a vital role in providing high-quality care to children and young people with complex medical needs in their homes or community settings.
Key Responsibilities- Provide skilled and high-quality care to children and young people with complex medical needs, following individualized care plans developed by the nursing team.
- Carry out delegated clinical tasks, including support for children with complex health needs and those dependent on technology.
- Work closely with families and the Children's Continuing Care Team to promote the child's or young person's well-being and provide a safe and supportive environment.
- Maintain accurate records and report any changes in the child's or young person's condition or potential hazards in the environment.
- Participate in and complete an orientation and on-going training program to ensure competencies in specific clinical procedures.
We are looking for a candidate with:
- Good general education, including at least 4 GCSEs or equivalent.
- Numerate skills.
- NVQ level 2 in care or equivalent knowledge/experience.
- Demonstrable experience of working with children and young people in a hospital or care environment.
- Effective written and verbal skills.
