**Role Overview:** Health and Wellbeing Practitioner - Prison Settings

The Forward Trust is seeking a skilled Health and Wellbeing Practitioner to join our team in the prison settings. As a Health and Wellbeing Practitioner, you will be working closely with prisoners aged 18 and over, providing support and guidance as they navigate their recovery journey.

Key Responsibilities:

  • Delivering wellbeing & recovery interventions and structured programmes for service user cohorts across the region.
  • Carrying out comprehensive assessments and designing SMART care plans.
  • Facilitating therapeutic groups and promoting holistic recovery and health and wellbeing.
  • Collaborating with multidisciplinary teams to ensure seamless service delivery.

Requirements:

  • Experience of delivering structured interventions and facilitating therapeutic groups.
  • Knowledge of accredited treatment programmes.
  • A thorough knowledge of and commitment to the 12-step process of recovery from addiction.
  • A full UK driver's license.
