We are seeking a highly motivated and experienced Mental Health Nurse to join our All Age Autism Intensive Support Team in Coventry and Warwickshire. As a key member of our team, you will work closely with individuals with autism and mental health needs, providing clinical assessments, risk assessments, and autism-informed interventions to reduce the risk of hospital admission.
Main Responsibilities- Clinical assessments and risk assessments to identify individual needs and develop personalized care plans
- Delivering autism-informed interventions to reduce the risk of hospital admission and improve outcomes for individuals with autism and mental health needs
- Working collaboratively with multi-disciplinary teams, including occupational therapists, clinical psychologists, and therapy assistants, to provide comprehensive support to individuals and their families
- Providing individualized training to families and care providers to enhance their understanding and support of individuals with autism and mental health needs
- Mental health check-ins and regular reviews to ensure the effectiveness of care plans and make adjustments as needed
