About the Role
We are seeking a highly skilled and motivated Practice Nurse to join our dynamic nursing team at Seaside Medical Centre. As a Practice Nurse, you will play a vital role in providing high-quality nursing services to our diverse patient population.
Key Responsibilities
- To undertake a range of nursing assessments and provide appropriate care and treatment in conjunction with the GPs, Nurse Practitioners and other practice nurses or independently, in line with competence and need.
- To maintain or improve standards of care and be actively involved in identifying practice population needs.
- To demonstrate and apply knowledge related to the Quality and Outcomes Framework (QOF).
- To provide therapeutic interventions within a broader health promotion/public health strategy and enable patients to take action to meet their priorities for health.
- To help provide education and training regarding chronic clinical conditions as required.
- To provide specialist advice to patients and relatives in relation to COPD, Asthma and, where qualified, Diabetes chronic disease conditions.
- To undertake any further training required to ensure appropriate role development.
- To practice within the NMC Code of Professional Conduct and maintain accurate records.
Person Specification
- Qualifications
- Relevant nursing/health degree.
- Demonstrable commitment to professional development.
- Experience
- Registered nurse with a desire to work in General Practice.
- Knowledge and Skills
- Knowledge of needs of patients with long term conditions.
- Aware of accountability of own role and other roles in a nurse led service.
- Knowledge of health promotion strategies.
- Awareness of clinical governance issues in primary care.
- Knowledge of patient group directions and associated policy.
