- Develop and implement effective care plans in collaboration with registered clinicians and other healthcare professionals.
- Conduct home visits and provide wound care services in the community.
- Collaborate with multidisciplinary teams to ensure seamless patient care and support.
- Participate in clinical supervision and peer review to maintain high standards of practice.
- Qualification as an Assistant Practitioner or Nurse Associate.
- Experience working in a hospital or community setting within the NHS.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Flexibility to work in a fast-paced environment and adapt to changing priorities.
