Job Summary

We are seeking a highly motivated and experienced Clinical Assistant Practitioner to join our team at NELFT North East London Foundation Trust. As a key member of our Tissue Viability team, you will play a vital role in providing high-quality patient care and contributing to the development of our community-based services.

Main Responsibilities
  • Provide comprehensive and holistic assessments of patients with leg ulcers and other wound-related conditions.
  • Develop and implement effective care plans in collaboration with registered clinicians and other healthcare professionals.
  • Conduct home visits and provide wound care services in the community.
  • Collaborate with multidisciplinary teams to ensure seamless patient care and support.
  • Participate in clinical supervision and peer review to maintain high standards of practice.
Requirements
  • Qualification as an Assistant Practitioner or Nurse Associate.
  • Experience working in a hospital or community setting within the NHS.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work in a fast-paced environment and adapt to changing priorities.
