About the Role

We are seeking a skilled Community Psychiatric Nurse to join our team of healthcare professionals. As a Community Psychiatric Nurse, you will be responsible for providing high-quality care to patients in their own homes or in community settings.

Key Responsibilities
  • Conduct thorough assessments of patients to identify their mental health needs and develop personalized care plans.
  • Work collaboratively with multidisciplinary teams to ensure seamless care delivery and effective communication.
  • Provide education and support to patients and their families to promote mental health and well-being.
  • Participate in quality improvement initiatives to enhance patient outcomes and service delivery.
Requirements
  • Current registration with the Nursing and Midwifery Council (NMC).
  • Minimum of 6 months paid experience as a Community Psychiatric Nurse in the UK.
  • Full UK driving license and own vehicle.
