Job Title: Community Psychiatric Nurse

We are seeking a skilled Community Psychiatric Nurse to join our team at NHS Professionals Limited. As a Community Psychiatric Nurse, you will be responsible for providing high-quality care to patients in their own homes or in community settings.

About the Role:

This is a flexible role that offers a unique blend of professional fulfillment and adaptable scheduling. You will have the opportunity to work in diverse community settings, providing vital healthcare services and making a significant impact on patients' lives.

Key Responsibilities:
  • Providing robust assessment of patients who have been referred due to presenting with mental health difficulties.
  • Identifying appropriate care pathways and ensuring the least restrictive option to meet the needs of the patient and ensuring their well-being.
  • Signposting to appropriate care providers, including primary and tertiary care.
  • Raising concerns to senior staff for support and management.
  • Completing documentation and planning care for patients.
  • Presenting cases to Multi-Disciplinary Team for discussion around care pathway.
  • Liaison with colleagues, nursing, occupational therapy, medics, psychology, and admin.
Requirements:
  • A minimum of 6 months paid experience as a Community Psychiatric Nurse in an UK hospital environment within the last 3 years.
  • Comply with NMC Professional Registration and keep up to date with current nursing practices.
  • Full UK driving licence and own vehicle.
