We are seeking a Person-Centered Worker to join our team, providing tailored support to our service users and enhancing their overall quality of life.

Key Responsibilities
  1. Develop and implement personalized support plans that address individual needs and preferences.
  2. Support service users in accessing community resources and services.
  3. Maintain accurate records and communicate effectively with colleagues and stakeholders.
  4. Collaborate with healthcare professionals to ensure seamless care delivery.
Requirements
  • A compassionate and empathetic attitude, with excellent communication skills.
  • Ability to work independently, taking initiative and responsibility for tasks and projects.
  • Knowledge of relevant legislation and policies, including safeguarding and data protection.
  • Flexible and adaptable, with a positive attitude towards change.
