This is an exciting opportunity to work as a Respiratory Nurse Specialist with Chemistree Solutions Limited, delivering the highest quality of care to improve patient outcomes in line with local and national guidelines.
You will be working in primary care, delivering a unique nursing programme that focuses on driving best practice, delivering excellent patient care, and improving patient outcomes.
We are looking for a patient-focused NMC registered nurse who can demonstrate experience of working in Asthma and COPD. Excellent planning & organizational skills along with confident driving ability are required.
Key Responsibilities:- Conduct COPD/Asthma patient reviews via telephone or video-consultation or in general practice where appropriate.
- Support healthcare professionals/patients to make informed decisions about treatment pathways in Asthma and COPD.
- Work in partnership with the NHS in primary care, delivering a nursing programme that focuses on driving best practice, delivering excellent patient care, and improving patient outcomes.
- This role requires excellent planning & organizational skills, confident driving, and flexibility in relation to travel and occasional overnight stays.
